Continuity of Care RARE Patient Impact Grant

Avery’s Hope is now able to offer financial support to rare pediatric gastrointestinal (GI) disease patient families in need during the COVID-19 crisis. This project was made possible by a grant from Global Genes.
Priority areas of approved financial support include patient access to distance learning support, including tutoring, telemedicine and career counseling resources (i.e. internet access, smart phone/table access, etc.) and basic living expenses such as rent and mortgage, and transportation to and from medical appointments.
As per the grant guidelines, funds granted may not be used for medical expenses such as medical or dental bills, insurance premiums or copayments, deductibles, medications, and the like.
All information will be used solely for statistical purposes only. No personal information will be shared. This grant is open to all rare pediatric GI disease patient families without discrimination.
To apply for financial assistance, please fill out the following form. Grants will be awarded through December 1, 2020 or until funds have been exhausted. Grants will range from $200 - $2500 dependent on need.
